care agency in Northern California. Our agency specializes in
delivering comprehensive in-home care and case management for older
adults, children, and those living with disabilities across many

relocation expenses. Type of Employment Work Schedule: This is a
full-time, exempt position with a standard work schedule of Monday
through Friday, 8:00 AM to 5:00 PM. While the role generally
follows these hours, flexibility may be required to meet business
needs, including occasional evenings or weekends. Who You Are: You
are a motivated Physical Therapist with proven success in a
supervisory role, who shares our vision of delivering in-home care.
You treat patients in a caring and compassionate manner and help
them achieve their goals of staying in their own home whilst
receiving treatment. Duties and Responsibilities: To perform this
job successfully, you must be able to perform the following duties
successfully: Plans, develops, organizes, implements, evaluates and
directs the overall operation of the rehabilitation department as
well as its programs and activities, in accordance with current
state and federal laws and regulations; and respective practice
act(s) in the state. Identifies, in conjunction with the Operations
Manager, the departments key performance indicators. Establish an
ongoing system to monitor these key indicators such as the Quality
Assurance and Performance Improvement process in the rehabilitation
department. Develops and implements processes and systems to
effectively lead, guide and direct the delivery of rehabilitation
services including but not limited to processes for the timely
screening, evaluation, treatment, discharge and all associated
documentation requirements for residents in the facility. Ensures
that all therapists are appropriately documenting, billing and
coding for patients receiving therapy. Serves as the companys
rehabilitation representative and spokesperson to the public as
well as in the professional arenas. In so doing, must periodically
deliver a confident, concise, message on a variety of topics that
is understood by a wide range of people with diverse backgrounds
and levels of education. Develops and maintains positive
relationships with patients, employees, and family members through
scheduled, deliberate positive interactions with them using face to
face, phone, or other means of communication. Identifies and
collaborates with members of the interdisciplinary team,
physicians, consultants, and community agencies to identify
opportunities for enhanced services to the patients and/or resolve
issues. Essential Requirements: A masters degree in physical
therapy Minimum of 3 years experience in a supervisory role as a
home health care provider or similar role. Current California State
license as Physical Therapist Current CPR certification Valid
California Drivers License Eligible to be insured under our
liability policy with a clean DMV report Your own car, and proof of
current auto insurance Required Skills and Abilities: Demonstrated
technical competence and accountable for work output. Demonstrates
proficiency in performing physical therapy procedures and
modalities and in assessing the effects of these Demonstrates
sensitivity with patients, family, and their medical team Ability
to work with others, at all levels within the organization, and
collaborate effectively. Above-average interpersonal,
problem-solving, and written and oral communication skills.
Professional behavior when on duty Ability to maintain patient
records and able to organize daily schedule Physical Requirements:
Sits, stands, bends, lifts, walks, and moves intermittently during
working hours. Able to lift 50 pounds without assistance. Able to
drive intermittently on a daily basis Compensation and Benefits:
Pay range is between $135,200 to $156,000 per year. Exact
compensation may vary based on skills, experience, and location.
You will be reimbursed for the mileage you travel from the office
to participants homes. You will receive a monthly phone stipend. On
the first of the month following 2 months of employment you will be
eligible for health insurance, dental insurance, vision insurance,

Spending Account (FSA). We offer 401(k) matching up to 4% after 1
year of employment and 1,000 hours worked Leave accrues starting on
